The incidence of a metastatic tumor the calvaria is uncommon, but it has considerable impact in field neurosurgery. hematogenous spread was common leiomyosarcoma. Calvaria one most uncommon locations for this type cancer. A 70-year-old man with history rhabdomyosarcoma and thyroid cancer came parietal mass that rapidly enlarged, followed by sudden hemiparesis severe cognitive impairment. Head CT scan revealed destructive soft tissue mass, indicating development effect pushing brain parenchymal slight compression right lateral ventricle midline deviation 8 mm to left side. Perioperative embolization done, effectively excised through extensive excision. Histopathology indicated malignant spindle cell mesenchymal tumor, After operation, patient's neurological status stable, there no sign recurrence. Recommendation managing based on histopathological findings insufficient. information case report essential scholarly purposes participant gave written informed consent publication. For treatment metastases skull, complete excision osteolytic lesions indicated.
